The clauses at a glance
| Clause | Topic | Evidence |
|---|---|---|
| 4 | Context | Interested-parties register, scope statement, needs/expectations |
| 5 | Leadership | OHS policy, roles/responsibilities, worker consultation log |
| 6 | Planning | Hazard ID register, legal register, OHS objectives |
| 7 | Support | Competence matrix, communication log, documented information |
| 8 | Operation | Procedures as workflows, change management, emergency plan + drills |
| 9 | Performance eval | Monitoring plan, internal audit, management review |
| 10 | Improvement | Nonconformity + CAPA, continual improvement register |
The three clauses most programs fail
- 5.4 Worker consultation — consultation occurs, but the log is thin. Aim for named attendees, topics, outcomes, and a linked action for each meeting.
- 6.1.3 Legal and other requirements — a spreadsheet is not a live evaluation. Link each requirement to the module (or control) that demonstrates compliance, and review quarterly.
- 9.3 Management review — minutes exist, but rarely map to the inputs/outputs the clause demands. A management-review template with sections per required input is the cheapest fix.
