Entry permit — continuous monitoring log
Attendant / entrant scan log
Sample records (pre-seeded)
| Permit # | Space | Last O₂ | Last LEL | H₂S | Attendant |
|---|---|---|---|---|---|
| CSE-221 | Vessel V-04 | 20.8 % | 0 % | 0 ppm | K. Brown |
| CSE-222 | Manhole MH-12 | 20.9 % | 0 % | 0 ppm | N. Silva |
| CSE-223 | Sump S-02 | 20.5 % | 2 % | 1 ppm | J. Tan — suspended |
Typical configuration
- Guard: entry blocked unless rescue plan attached and entrant-training check passes.
- Guard: LEL > 10 %, O₂ < 19.5 % or > 23.5 %, or H₂S > 10 ppm suspends the permit and triggers the escalation cron.
- Capability: continuous-monitoring log accepts readings every 15 min (configurable) and flags gaps.
- Effect: emergency escalation dials the named emergency contact for the site via the notifications module.
